Then, we'll build a detailed profile tailored to your needs and services. We also validate insurance coverage at this time. 

The final phase of this step is when we head back to your home to gather any missing details or information to better address your needs. 


What A Typical Day With Self-Care Would Look Like

Our primary concern for you as a patient is to ensure you are comfortable at home. One of our healthcare supervisors will shadow you in your home to ensure everything is taken care of properly.

Through "Family Room," you, your caregivers and healthcare professionals, and your family members can access details and information about your healing and recovery process! 

Once you permit specific individuals, they can access treatment information at any given moment. The report includes care plan details, doctor visits, progress, and so much more. All the data is conveniently located in one place to make sharing easy for you! Our "Family Room" platform helps keep everyone informed and on the same page - always.
